Most of our clients are referred to us by a healthcare professional, social worker, doctor or family member who recognizes that extra support is needed. This referral form is the first step in opening the door to help.

Once received, our team reaches out personally to learn more about the situation and set up our Care Discovery Meeting. From the very first conversation, our focus is on compassion, clarity, and trust to make sure that everyone involved feels supported and informed from the start.

Step 2: The Care Discovery

In this in-depth conversation, we take the time to get to know you.

Learn your family dynamics, care concerns, emotional challenges, and financial picture.

We uncover what’s most important to you so that every decision made moving forward reflects your unique values and priorities.

Step 3: Golden Path Plan

Your personalized roadmap for the journey ahead.


 After your Care Discovery, our team meets internally to review your situation in full. Medically, emotionally, and financially. Together, we create a customized care strategy designed to help you move forward with clarity and confidence.


Your personal Golden Concierge pricing package will also be presented at this stage, clearly outlining the services, scope, and support you’ll receive so there are no surprises along the way.
